The venue has been closed and reopened several times. The theater opened its doors to the public for the first time on the night of February 14, 1921 to reveal the artistic creation of the famous Italian artist, Thomas W. Lamb. During the first decade the theater continued to host Broadway plays. It was in 1935 that the décor was given an Art Deco style and the name was changed to Mayfair Casino. The idea behind it was to ultimately make it a casino, but as gambling was not permitted the idea could not be given practical implementation. The Mayfair Casino eventually had to close down a year later.

Again efforts were made to turn Ohio Theatre Playhouse Square Cleveland into a movie theater. In 1943 the theater began showing movies. However, the tragic incidence of a lobby fire in 1964 damaged the beautiful interior of the theater. Attempts to hide the smoke damage were made by repainting the lobby. Eventually the theater again closed in 1969. They were rumors of closing it down for good during the years that followed but public disapproval prevented it from happening.

It was in 1982 that the Ohio Theater opened again after extensive restoration work. According to estimations the renovation cost an amount of almost four million dollars. Ohio Theater was the first one of Playhouse Square Theater to reopen again. It boasted of an intimate venue that was a 1000 seat facility.

From the day it reopened till today, the theater has been providing quality entertainment. The intimate seating provides a wonderful setting for an acoustic show. It hosts theatrical productions and musical concerts ranging from opera to rock and it all takes places inside the walls of the theater that still stands as one of the landmarks of Cleveland.
